About Ben Hutchinson About

Ben Hutchinson is a qualified National HSEQ Manager, fatigue specialist and exercise physiologist. His work is grounded in adaptive and systems principles, including system safety, Resilience Engineering, STAMP/STPA, HRO, Human Factors, Safety-II and Safety Differently. He is currently completing a part-time PhD in Safety Science.

His interests span major hazard events such as gas and mining explosions and fatal construction incidents, safety-in-design, Major Hazard Facilities, process safety, fatigue and organisational culture. He also explores complex adaptive systems, organisational drift, HRO, resilience engineering and the psychology of risk to help prevent major incidents in complex environments.

Ben Hutchinson applies bow-ties and systemic models such as FRAM, AcciMap and STAMP for both reactive and proactive risk management. Above all, he studies how people and teams make sense of situations, make decisions and construct a social view of risk. He champions a humanistic approach that respects adaptive intelligence and builds relationships through dialogue rather than control.
